When entering the ICT/Laptop/Telephony page you will see the below shown form that incorporates all the elements required to be able to request ICT/Laptop/Telephony Services.

Fields:

Name: Enter your name and Surname

Email: Enter your email address (Required)

Please Indicate Location: Identify the location where the Hardware is stationed.

Message: Clearly indicate the issue in question. This is a free text field. Please try and enter as much detail as possible.

Once all the details are entered as per example below click on the SUBMIT REQUEST button in Red.
